Incident Investigation / Root Cause Analysis Course

Things happen every day in the industry that are out of the ordinary, unexpected and that should not have happened. Most times these events are nothing more than an inconvenience to our schedules but sometimes these events cause major problems that can lead to a major catastrophe and even disaster.

This course details the processes involved in the investigation of these “incidents,” and shows the participants how to categorize them as “near misses,” “minor” or “serious” events. It further details ways to find the root cause of the incident and how to possibly prevent it from happening again. After going through the investigative basics of determining, who, what, when, how and why, the participants are walked through a series of case studies ranging from ISM non-conformity to major shipwrecks and oil spills to learn how to analyze the event and determine the ultimate root cause.
